India is raping moving toward Digitalization. So, mostly form is submitted online on the portal by coordination of Employee and Employer or EPF Officials. Relevant Forms are provided under the EPF & MP Act, 1952. Use the appropriate form for claiming Provident Fund Pension, withdrawal benefit/scheme certificate, Employees' Deposit Linked Insurance benefit, etc. as given below
- Nomination Form No. 2 & also Declaration Forms in Form No.11 (Revised) of the New Employees.
- Receipt of PF transfer forms (Form 13) from new appointees. Send Form 13 to R P F C/Ex- Employers trust to get fund transferred through the concerned R P F C office.
- Receipt of Form 31 for Advance from EPF Account
- Receipt of forms (EPF Composite form) after 60 days of leaving of the employee making full & final settlement.
- These days for PF withdrawal without tax deduction one required to fill Form 15G/15H
- Submitting Form 9 revised. This Form submitted immediately after coverage, within 15 days of coverage. However, Establishment is covered under EPF & PF Act after 01-10-2017 is not liable to submit the Form 9 revised as per the notification issued from EPFO.
- Form 5A to forward immediately after coverage. Whenever, in case of the change in the ownership, it has to be intimated within 15 days of the change.
- Form 12A required to submit monthly by 25th of following month.
All Application must be signed and attested by Employer.
All necessary forms and documents can be prepared/drafted by us to provide Employer/Employee and the same can be submitted at EPF offices by our field Executives. We also deal with EPF Officials on behalf of Employer as representative if required in some cases.
